Build a differential before choosing an answer.
How to approach this subject
Internal medicine questions ask you to connect a presentation with physiology, investigations and the next decision. Start by summarizing the patient in one sentence: age group, time course, principal problem and the most discriminating findings. Separate the likely diagnosis from the action the question actually requests. A correct diagnosis does not automatically identify the best investigation or immediate management step.
When reviewing an answer, identify the clue that makes the preferred option stronger than its closest alternative. Then ask what change in the vignette would make that alternative correct. Organize missed items by the reasoning step you missed: recognizing the syndrome, interpreting a test, assessing urgency or choosing management. Revisit those items after studying the relevant system, then mix systems to practice deciding which framework applies.
